Management of Medical Conditions in School

If your student has a chronic condition that requires intervention during the school day, a Medical Management Plan needs to be on file so that we can best meet their needs.  Below, you will find plans that can be printed off and taken to your provider if you would like to use them.

In order for your student to receive any medications at school, we must have one of these forms on file, signed by both the provider and parent.  These forms are good for one school year.
