All enrollments need to include the following with the basic application to enroll in Eaton Rapids Public Schools:  Please either sign the document digitally (this can be done with Adobe Reader, or using a similar online pdf editor such as DocHub) or print out the filled-in form and sign it by hand. Enrollment forms are not complete without a signature in the signature fields. 
 
Up-to-Date Immunization Record or Waiver from the Health Department Michigan law requires the enrolling adult to provide a current list of the student's vaccination status.  It is the responsibility of the parent or enrolling adult to obtain this information from the doctor or health department where the student received his/her vaccinations. Students will not be allowed to attend school without an up-to-date vaccination record or waiver from the health department on file. Parent/Guardian Identification & Custody Documents The enrolling parent/guardian must supply a photo ID.  If the enrolling adult is not listed on the certified birth certificate, we require the court order establishing the right of custody.  
 
Residency Requirement   Two proofs of residency showing the parent/guardian's home address i.e. utility bill, property tax bill, or a copy of a rental/lease agreement. The photo ID can be one of your proofs if the address matches the enrollment forms.  If the student and their family is living with friends or relatives, the person who rents or owns the home, trailer, or apartment must sign an affidavit affirming that the student shares the home.
 
If you reside outside of the Eaton Rapids Public Schools district:
        • Schools of Choice window is OPEN: Submit and be accepted as a Schools of Choice student Schools of Choice acceptance is not automatic.  You will be notified of the district decision by the enrollment coordinator.  After you are notified of acceptance, you will then need to submit the enrollment forms and supporting documentation. 
        • Schools of Choice window is CLOSED: When the Schools of Choice window is closed, students not living in our district are still welcome to apply to the district.  Admission is not guaranteed. The parent/guardian must fill out the enrollment paperwork and obtain a release from their resident district. Please contact your resident district and have them forward the release to the ERPS enrollment coordinator.  The enrollment coordinator will notify the parent/guardian of the decision of the district.
 
Birth Certificate You will need to show the enrollment office the student's certified birth certificate (birth certificates do not have to be from the United States). If you do not have one, talk to the Enrollment Coordinator about your circumstances. Michigan law requires the enrolling adult to provide this. If you do not have a copy, your student will be able to start school but you must present a copy of the original birth certificate within 30 days.  If not presented within 30 days of enrollment, the district must notify local law enforcement in accordance with the Missing Child Act. Additional Requirements for Kindergarten and Developmental Kindergarten students
 
Hearing and Vision screening tests are required in order to start attending any Kindergarten program. All tests must be done within 6 months of the start of school.  The Barry-Eaton County or Ingham County Health Departments provide free hearing and vision tests for children starting kindergarten. Call BECHD at 517-541-2630 or ICHD at 517-887-4311 for more information. Call early!  Screenings at BECHD only occur on one day every other month.  Screenings from your child's primary care physician is acceptable as well.  
 
Kindergarten Oral Health Assessment Law (Public Health Code Act 368 Section 333.9316) was passed to ensure that children entering their first year of school are able to receive an oral health assessment (dental screening) prior to starting school.  Good oral health is important to help children stay healthy and ready to learn.  This required assessment will let you know if your child has any dental problems that require attention by a dentist.  The assessment must be conducted by a Registered Dental Hygienist, Dentist or Dental Therapist.  All assessments must be completed within 6 months of the start of school.
 
Age Waiver For the 2025-2026 school year, students who are not aged five as of Sept 1, 2025, but who will turn 5 between September 2 and December 1 may still be enrolled with a waiver completed by the parent/guardian and submitted with the Enrollment Packet.  The building principal must sign the form.  All requests must be submitted by August 1st.
